Candidate Registration Form — Please print this out, complete it and return it to your instructor.
Copy of Baptismal Certificate — All candidates must contact the parish where they were baptized to request that a copy of their baptismal certificate be sent to Corpus Christi. You can search online for the contact information for a parish other than Corpus Christi.
(If you were baptized or received your first communion at Corpus Christi OR if you submitted a copy of your Baptismal Certificate as part of school registration, you do not need to provide another copy.)
Statement of Catholic Standing — All sponsors must contact their parish office to tell them that they intend to be a Confirmation sponsor so they need verification that they are a practicing Catholic. Parishes do this all of the time. Your parish will know exactly what you are requesting. Please have the document provided by your parish sent to Corpus Christi. As a reminder, here are the requirements for being a Confirmation sponsor.
(Sponsors who are parishioners at Corpus Christi do not need to provide this information. Also, sponsors do not need to provide a copy of their Baptismal Certificate.)
Permission to be Confirmed at Corpus Christi (for non-parishioners only) — If you are not a member of Corpus Christi parish, you will need to ask your pastor to send a letter to Fr. Mark granting permission for the candidate to be confirmed at Corpus Christi. If you would like to become a member of Corpus Christi Parish, please obtain a parish registration form from the parish office.
