What are all of the documents that I need to submit?

If we submitted a Baptismal Certificate for school registration or a prior sacrament, do we need to provide another copy?
No, you do not need to provide an additional copy if you have submitted one earlier for the above reasons.
What are the requirements for being a sponsor?
Here are the guidelines on being a sponsor from the Diocese of Colorado Springs:
Those who serve as sponsors of confirmands should:
1. Have received all 3 sacraments of initiation;
2. Be practicing Catholics who participate in Mass on Sundays and Holydays of obligation and receive the Sacraments of the Eucharist and Reconciliation
regularly;
3. Be living their Christian vocations as either a single person or one who is validly married in the Church;
4. Believe all the Catholic Church teaches and believes and strive to live an upright life;
5. NOT be the parent (or adoptive parent) of the one to be confirmed;
6. Be at least 16 years of age;
7. Understand and accept their great responsibility before God and the Church in becoming sponsors.
8. It is fitting (but not required) that the sponsors (godparent) of Baptism be also the sponsor of Confirmation.
